 健常者と同じく、ポリオ体験者は、死ぬまでに、様々な原因により、体のどこかの痛みを訴えるものです。
 Polio survivors, not unlike the general population, will likely experience pain at some points in their lives due to a myriad of reasons.

 しかし、ポリオ体験者の場合、他の人達と違って、ポリオによる慢性的なマヒ部分が痛むことが多いようです。
Unlike the general population, however, polio survivors are somewhat more likely to experience pain as a consequence of the residual paralysis or paresis of chronic polio.

 仮にポリオ体験者が痛みを感じたとしても、それが直ちにポストポリオのせいだとは、必ずしも言えません。
If a polio survivor is experiencing pain, this does not necessarily mean that the pain is a symptom of post-polio syndrome.

 初期的な診察の参考にはなりますが、ポリオ体験者に痛みがあったからと言って、それがポストポリオの症状だと断定することはできません。 
Taking this yet one step further, even if a polio survivor has been appropriately diagnosed with post-polio syndrome and is experiencing pain, this does not necessarily mean that the pain is due to the symptoms of post-polio syndrome.

 痛みがあれば、適当な医学的評価を受けることが大事です。痛みは、とても良性のものから深刻なものまで、幅広いケースがあります。
If pain is being experienced, it is essential that an appropriate medical evaluation be made because the pain could be due to any number of factors ranging from very benign to quite serious.

 ポリオ体験者の痛みの診察に当たって、最初にしなければならないのは、患者がポストポリオにかかっており、その痛みがそのせいかどうかを見極めることです。次に、痛みの症状が体験しているポストポリオの一部なのかどうかを見ることです。
The first step in assessing pain in a polio survivor is determining if a diagnosis of post-polio syndrome is appropriate and secondly if the pain symptoms are a part of the post-polio syndrome being experienced.

Post-polio syndrome is diagnosed clinically and, unfortunately, no clinic test is specific to detect this syndrome.

Therefore, the person experiencing the pain should be evaluated by a physician who has experience in managing and diagnosing post-polio syndrome.

If the pain symptoms are due to the effects of post-polio syndrome, pain is most likely due to overuse of muscles, tendons, ligaments and/or joints.

Other problems that can occur with post-polio syndrome are secondary nerve compression syndromes, commonly at the wrist and occasionally at the elbows.

Typical pain syndromes associated with post-polio late effects include muscle pain and cramping.

The patients may describe fasciculations (a crawling sensation) which is exacerbated by pain, physical activities, stress and occasionally cold weather.

 筋肉の過用(使いすぎ)と、筋性の(ミオゲンの)痛みが起こるかもしれません。そして、これも体の使いすぎによるのです。
Overuse of muscles and direct myogenic pain may be occurring and, again this is secondary to physical overuse.

Typically, the myogenic pain and fasciculations will decrease or disappear entirely with rest.

 温めたり、静かにストレッチングをしたり、軽いマッサージをすることが、治療の一助になります。
Occasionally heat, gentle stretching and light massage are useful adjunctive treatments as well.

 緊張することが、体を傷つけることにつながることもあります。
Strain injuries also are not uncommon.

Areas affected include muscles, tendons, bursa and ligaments.

These strain injuries may occur chronically or acutely.

They may be posturally related or occurring as a result of overuse of arms, shoulders and lower extremities.

Pain from the shoulders resulting from supraspinatus or occasional biceps tendinitis are not uncommon.

Elbow pain and knee pain from progressive genu recurvatum is not uncommon.

 前反膝は、膝の周りの筋肉と靱帯が弱くなり、逆向きに曲がるようになってしまう変性が進み、関節の後ろの部分などの痛みが強くなるものです。

Genu recurvatum is a condition in which, because of weakness of the ligaments and muscles around the knee, there is progressive backward deformity of the knee, resulting in progressive pain, usually the back of the joint.

 これらの傷害は、補装具や松葉杖を用いた物理的な治療に加え、慢性的なストレスを取り除くことによって、減らすことができます。
These injuries and symptoms can be controlled by eliminating chronic stressors which may, in addition to symptomatic treatment, consist of protecting the joints through bracing, or decrease in crutch walking.

 よく見られるもう一つの問題は、関節の老化に伴う病気です。
Another problem frequently seen is degenerative joint disease.

 慢性的なストレスを持ちながら歩く患者は、早く老化する可能性があります。
Degenerative changes are potentiated if the patient has been walking on unprotected joints with chronic abnormal stresses.

 適切な補装具の使用や、姿勢や椅子の変更だけでも、 症状が改善されることがあります。
Typically, symptoms are improved by improving support with appropriate bracing, postural modification and improved seating support.

 松葉杖や車椅子を使うポリオ世代には、神経圧縮症候群もはやっています。
Nerve compression syndromes are much more prevalent in the polio population in those who are crutch or wheelchair ambulators.

 同年代の健常者と比べて、手首の手根トンネルと尺骨神経の圧縮が、約4倍も多く見られます。
Carpal tunnel and ulnar nerve compression at the wrists are four times more prevalent in this group of polio survivors than in the general population.

 慢性的なストレス、松葉杖での歩行、手動式車椅子での移動は、症状を悪化させる要因です。
Chronic stressors, crutch walking and manual wheelchair propulsion are the primary aggravating factors.

 松葉杖から動力式カートに変えたり、手首の位置を変えて手の広がりを狭くして休めるこことで、軽減できます。
These are alleviated by reducing the stress on affected areas by use of power carts, thereby reducing crutch walking, and by the use of resting hand splints to provide better protection and positioning of the wrist.

 ポリオ体験者には、神経根障害も見られます。特に、重症の脊柱側湾症の人や、慢性的に体力が低下し、きちんとした姿勢を保っていられず、背中や後腰部の延びきった人に多いです。
Radiculopathy may be a factor in some polio survivors, particularly those who have severe scoliosis or neck or low back hyperextension through weakness and chronically abnormal posture.

If degenerative changes are present in the spine, this also is a factor.

 これは、姿勢を変えることで改善できます。
This may be relieved by decreasing abnormal postures.

 コルセットや補装具の使用も、試してみると良いでしょう。
If a body corset or body brace is not being worn, this may be an option in some cases.

Improved seating positioning will also decrease symptoms sometimes in these cases.

In other cases, traction may be indicated.

Symptomatic treatment with medications and therapeutic modalities is also a benefit in many cases.

As can be surmised, polio survivors can experience pain as a result of a large number of factors.

The first step in the treatment of pain should be in accurately diagnosing the cause of the pain.

It is only at this point that an effective strategy for managing the pain can be developed.
