　健康やリハビリの方法を手に入れるのは、一種の「買い物」だと思いませんか？
Have you ever considered shopping for your health and rehabilitation?

　車を修理するときの保険と比べてみてください。
Imagine for a moment that you have insurance for the repairs on your automobile.

　愛車は、６５年前の製造だとします。
Then imagine that your car is 65 years old!

　街中を歩き回って、番経験豊かな修理工を探すでしょう。
You take time to shop around and find the most experienced mechanic in the area.

　街中を探し回ってでも、本当にその人に直して欲しくなるでしょう。
Even if it means driving all the way across town, that is the person you want taking care of your car.

　では、信頼できる修理工が見つかったとします、でも、その人に頼むと、修理費がいくらになるか、全然予想もつかないとしたら。
Now imagine, after finding a good mechanic you trust and one that gives you great service, you are allowed only "X" number of insurance dollars for that one mechanic regardless of what repairs are needed.

　年の初めに修理したばかりなのに、半年くらいでまた直さなければならないとしたら。
If you have your car repaired in the beginning of the year and you need it repaired again the middle or end of the year,

　別の修理工を探さざるを得ないでしょう。信頼に見合っただけのお金を払ってしまったのですから。
you will have to go to another mechanic because you have already spent the allowable dollars with the mechanic you have come to trust.

　それは、多分もっと経験が浅く、未熟で、良い道具を持っていない修理工に切り変えることを意味します。
This means you have to transfer to someone who is perhaps, less qualified, experienced, or has less sophisticated equipment.

　では、車の例をリハビリに当てはめてみたらどうでしょうか。
Now, you say what does this have to do with rehabilitation?

　１９９７年予算均衡法（ＢＢＡ）が、１９９９年１月から、医療界にも施行されました。
Starting January of 1999, the Balanced Budget Act (BBA) of 1997 went into effect for Medicare.

　そのため、外来のリハビリでは、１人１年間の治療費が、１，５００ドル以下に抑えられることになりました。
What this does is limit Medicare recipients to $1,500 per calendar year per out patient rehabilitation facility.

　病状や回数毎ではなく、１年毎、病院毎にです。
Not per injury or event, but per year / per facility.

　そのサービスには、理学療法(PT)、言語療法(ST) 、作業療法(OT)も含まれます。
These services include physical therapy (PT), speech therapy (ST) and occupational therapy (OT).

　予算均衡法（ＢＢＡ）では、理学療法(PT)、言語療法(ST) の金額が合算されます。
In the BBA, the dollar limit has combined physical therapy and speech therapy.

　作業療法は、理学療法(PT)と会話療法(ST)の合計学とは、別の経理になります。
Occupational therapy has the dame dollar limit separate from PT/ST.

　運悪く、事故や病気でリハビリを受け、その額の上限を年の途中で使い切ってしまえば、その後の症状に対する財政的支援は受けられません。
If you are unfortunate enough to suffer an accident, illness or disease requiring rehabilitation services which exhausts this limit and you have the misfortune to undergo another complication in the middle or end of the year, you have no additional funds for the provider with whom you established a trusted relationship.

　理学療法は、健康についての動態的な職業です。
Physical therapy is a dynamic health profession.

　医師は、医学的な手法によって、一人一人、年令に応じた診断と治療を行います。
Practitioners are involved in evaluating and treating individuals of all ages by physical means.

　熱、水（アクアセラピー）、運動、音、電気は、個々の機能を回復する方法の選択肢の例です。
Heat, water (aquatic therapy), exercise, sound and electricity are some examples of means to restore optimal functional independence.

　理学療法は、短期的又は長期的な障害を持つ患者の、治療の中心です。
Physical therapy is a vital part of the total care for patients who have problems of temporary or permanent disability.

　怪我で痛い伴う人もいますし、手足がない人もいます。
Some may have pain due to injury, disease, or loss of limb.

　会話療法は、コミュニケーションを取れない人が、日常生活を改善しようとする時に、有効な治療法です。
Speech therapy is an effective profession for those patients who need to improved communication skills so that they are able to function in daily life situations.

　患者は、おのおのの障害に応じて、治療が受けられます。
The patient is assisted in adapting to his or her disability.

　治療は、発音不明瞭、不全失語症、声の不調、えん下困難、口と顔の周りの異常といったようなものに対する治療を含みます。
The treatment itself may include such areas as articulation disorders, dysphagia, voice disorders, swallowing disorders, and oral-facial abnormalities.

　加えて、コミュニケーションの方法を広げたり、代わりの役を果たす方法を覚える必要もあります。
In addition, augmentative and alternate communicative systems are utilized.

　作業療法は、最終的に、体の機能を最大限に伸ばそうとします。
Finally, occupational therapy services promote the attainment or enhancement of maximum functional abilities in patients' daily life tasks.

　限界はありますが、作業療法は、動態的な治療を通じて、評価、対象物の操作、セルフケアの促進、自立の支援などを行います。
Occupational therapy includes, but is not limited to, such techniques as assessment, manipulation of objects, and the development of self-care and capacity for independence through the dynamics performance.

　フロリダ州シトラス郡には、病院に付属しないリハビリセンターが、約１０カ所あります。
In Citrus County [Florida] there are approximately ten different rehabilitation centers, not including the hospital based programs.

　それぞれ、少しずつ異なった治療法を施しています。
All of these centers have a little something different to offer.

　医師の処方箋は、自分が一番良いと思う薬剤師に持っていけば良いものです。
The prescription you receive from your physician for your medication can be taken to any pharmacist you prefer and trust.

　リハビリの処方箋も、同じです。
The same is true for your prescription for rehabilitation.

　どの医学的決定でもそうですが、リハビリの場所も、よく考えて決めなければなりません。
This, like all medical decisions, should be an educated decision.

　この文章の最初にも書きましたが、医者を選んだり、車を買うディーラーを選んだりするように、療法士も選ぶべきです。
You should shop for your therapist, just like you did for your doctor, just like you did for that mechanic for your car in the introduction to this article.

　健康は、車より大事なはずです。
Your health is even more important than your automobile.

　設備やサービスの質を見たり、療法士や他の職員と話しをしてみるために、施設を何カ所か見て歩いた方が良いです。
Take the time to go and visit several facilities, see what each has to offer in equipment and service, talk to the therapist and staff.

　そこでリハビリをするかどうかの相談に行こうとしても、ゆっくり話しができないような施設であれば、実際にリハビリに通っても、ゆっくり相談にのってもらえないことでしょう。
If they don't have the time to speak with you before making an appointment, chances are they won't have time during the rehabilitation process, either.
