 年を取ると、体力が落ちるのは当然です。
As everybody grows old it is natural that physical limitations increase.

 ポリオ体験者の体力低下は、他の人よりもずっと早く起こります。
 体力をできるだけ残し、なるべく病院のお世話にならないようにすることが大事です。
With polios, the limitations come much earlier and, in these days, it is necessary to cope to the best of our ability and without much assistance from 'medical profession.'

 私はこの数年間、できることが減って、疲れやすくなりました。
During the last few years, I find myself being able to do less and less and tire far too easily.

 ポリオ関係のホームページの中で一番良いと思われる、リハビリ・ガゼットを読んでいる方には、地元で病気とポリオの関連について詳しい医師の名前を教えて欲しいと、御願いしています。
I would, therefore, like to suggest that readers of Rehabilitation Gazette, which seems to be the last polio link, submit names of doctors they know in their home town who can easily relate medical problems to polio problems.

Larry Schneider in Rehabilitation Gazette (Vol 22, 1979)

 ガゼット・インターナショナル・ネットワーク研究所(GINI)の創設者である、ジニ・ローリー氏は、ポリオ体験者からの強いお願いに応えて、その人達と、ベンチレーター(人工呼吸器)使用者と、政治家と、医師と、産業界の代表を集め、ポリオ体験者の高齢化の問題について研究しています。

Gini Laurie, founder of Gazette International Networking Institute (GINI), responded to the polio survivor's plea and gathered together polio survivors, ventilator users, policymakers, physicians, and representatives from industry to explore the needs of aging polio survivors.

 1981年のシカゴ会議は成功し、全米に知らせる努力は、現在国際ポリオネットワーク (IPN)と呼ばれているものに結実するための触媒になりました。
This successful 1981 Chicago meeting was the catalyst for the expansion of the educational efforts of the nationwide and worldwide network now known as International Polio Network (IPN).

Poliomyelitis Today

 ポリオ(脊髄性小児麻痺)は、ソーク (1955)とセービン (1962) のワクチンの接種が始まって以降、ほとんどの国で絶滅しました。
Poliomyelitis (infantile paralysis) has been eradicated from nearly every country in the world since the approval for use of the Salk (1955) and Sabin (1962) vaccines.

  1997年の1年間の、世界中のポリオ感染者数は5,185件と報告されました。1998年には、5,867件でした。
In 1997, 5,185 cases of polio were reported worldwide; in 1998, 5,867.

 この人数が増えたのは、調査技術の向上によるものです。
The increase in the number of cases is due to improved surveillance and reporting techniques.

 世界保健機構(WHO)は、実際の観戦者数は報告の5〜6倍に上ると推定しています。
The World Health Organization (WHO) estimates that there are five to six times as many paralytic cases as reports received.

 にもかかわらず、WHOは西暦2000年を、急性ポリオ根絶の目標年に据えました。
Nevertheless, WHO has targeted the year 2000 for the complete eradication of acute poliomyelitis.

 WHOは、ポリオで障害を受けた人が、世界中で1,200万人生存していると推定しています。
WHO estimates there are 12 million people worldwide with some degree of disability caused by poliomyelitis.

 1994年のアメリカ政府の保険予備調査によると、全米で約 100万人のポリオ体験者がいるとされています。
A 1994 National center for Health Statistics survey reported a preliminary estimate of one million survivors in the United States.

 そのうち約33,000人は、何らかのすマヒを持っているとのことです。
About 33,000 of them reported paralysis resulting in some form of impairment.

ポリオ体験者は、次のような症状を訴えることがあります。
Survivors of Poliomyelitis May Experience Symptoms
that Include:

・ 何となくの疲労感。筋肉の疲れ安さや、全身の疲弊感。
・ Unaccustomed fatigue - either rapid muscle tiring or feeling of total body exhaustion.

・ 新たな筋肉の虚脱感。初期の感染で損傷を受けた側だけではなく、受けていないと思われる側においても。
・ New weakness in muscles, both those originally affected and those seemingly unaffected.

・ 筋肉や関節の痛み。
・ Pain in muscles and/or joints.

・ 不眠。 
・ Sleeping problems.

・ 呼吸困難
・ Breathing difficulties.

・ えん下困難。
・ Swallowing problems.

・ 寒さに弱くなること。
・ Decreased ability to tolerate cold temperatures.

・ 歩行や入浴といった、日常生活動作ができなくなること。
・ Decline in ability to conduct customary daily activities such as walking, bathing, etc.

 これらの一般的な症状には、さまざまなレベルがあり、それもいつの間にか悪化します。
These general symptoms are experienced in varying degrees, and their progression can be insidious.

 単なる老化と見過ごすわけには行きません。
They should not be dismissed simply as signs of aging alone.

 最近の研究によると、ポリオにかかってから後遺症とともに生きてきた年数が長い人は、余命が短くなるリスクを負っているという結果が出ています。
Current research indicates that the length of time one has lived with the residuals of polio is as much a risk factor as chronological age.

 また、元々重いマヒにかかった後に、機能がとても大きく回復した人は、元々マヒが軽かった人よりも、大きな問題を抱えている、というのです。
It also appears that individuals who experienced the most severe original paralysis with the greatest functional recovery are having more problems now than others with less severe original involvement.

 ポストポリオ症候群だという診断は、他の病気の可能性を排除することにより得らると言っても良いでしょう。
The Diagnosis of Post-Polio Syndrome is One of Exclusion.

 明確な試験方法はありません。 
A definitive test is not yet available.

 ポリオ体験者と医療関係者と家族の間に、混乱と疑問が広がるのは、 用語の意味についての共通認識がないからです。
Some of the confusion and doubt among polio survivors, health professionals, and family members is due to the lack of common understanding of terminology.

 ポストポリオの問題は、次のような区分に分けて考えると分かりやすいです。この分類ははっきりしたものではなく、その境界はあいまいです。
It is helpful to think of post-polio problems in the following categories, remembering that the groupings are not distinct but parts of a continuum.

 一番大きく、幅広い区分は、「ポリオの遅発性の影響」又は「ポリオ後遺症」と呼ばれるものです。
The largest and most inclusive category is called
Late effects of Polio or Polio Sequelae

 これらは、ポリオのせいで起こる、慢性的な損傷に伴う、特別の新しい健康問題と定義されています。
and is defined as specific new health problems which result from polio-caused chronic impairments,

 例えば、関節を使いすぎての退行的な関節炎、手根骨症候群、その他の繰り返し運動による問題、腱鞘炎、滑液包炎、脱臼、体の不具合を補おうとするための関節の過剰負荷。
e.g., degenerative arthritis of overused joints, carpal tunnel syndrome and other repetitive motion problems, tendonitis, bursitis, failing joint fusions, overstressed joints due to compensatory body mechanics.?

 「ポストポリオ症候群」は、ポリオの遅発性の影響の中の一つ下の区分です。それは、神経学上の不調と定義付けされています。脱力感が高まったり、異常に筋肉が疲労することが特徴的です。我慢する力が衰えます。
Post-Polio Syndrome, a subcategory of the late effects
of polio, id defined as a neurological disorder characterized by increased weakness and/or abnormal muscle fatigability (decreased endurance).

 各種の症状の中には、新たな脱力感、激しい疲労感、我慢できない筋肉や関節の痛み、機能の減退を含みます。
The cluster of symptoms includes new weakness, intense fatigue, and pain in muscles and joints resulting in decreased endurance and diminished function.

 ほとんどの臨床医は、ポストポリオ症候群のために、次の診断基準を用いています。
Most clinicians use the following criteria to diagnose post-polio syndrome.

 明白な急性ポリオの既往症を持つこと。
Confirmed history of acute paralytic polio.

 臨床医の中には、当初のポリオとその後の変化を比較するために、筋電図 (EMG)を取る人もいます。
Some clinicians perform an electromyogram (EMG) to document changes compatible with prior polio.

 当初の感染後、15年以上の症状の安定が見られた後に、通常の疲労や筋肉の退化、痛みがある場合もない場合もあるが、新たな筋肉の疲労といった異常が出ること。
Recovery followed by 15 years or more of stability preceding the gradual or abrupt onset of new weakness and/or abnormal muscle fatigability, with or without generalized fatigue, muscle atrophy, and/or pain.

 上のリストにあるような問題を起こしても、他の原因の可能性を、排除しなければなりません。
Other conditions that might cause the problems listed
above must be excluded.

 新しい、ゆっくり進行する、神経学上の筋肉の虚弱化は、筋骨格的、または、疲れて壊れる問題です。
To distinguish new, slowly progressive muscle weakness that is neurological in origin from that which is due to musculoskeletal and/or
wear and tear problems,

 研究者はこれを、ポリオ後進行性筋萎縮症 (PPMA)と読んでいます。新たな虚弱化と筋肉の萎縮が、慢性的に進行することと定義されています。

researchers coined the phrase Progressive Post-Polio Muscular Atrophy (PPMA), defined as progressive new weakness and atrophy in muscles

with clinical or sub clinical signs of chronic partial denervation/reinnervation compatible with previous acute poliomyelitis.

The term is used less often today, giving way to post-polio syndrome.

 ポリオ体験者は、上に書かれたうちの、一つまたは複数の問題を感じています。なお、色々な名称があるからといって、あまり心配する必要はありません。
Polio survivors may experience one or more of the problems described above and should not become unnecessarily concerned about the label for their syndromes.

 もちろん、ポリオ体験者も、他の人たちと同じような健康上の問題を抱えています。
Polio survivors may, of course experience the same health difficulties as everyone else,

 ポストポリオに似たものもありますし、最初のポリオのせいで、慢性的に神経が損傷を受けているせいのものもあります。
some with symptoms mimicking post-polio syndrome, some magnified due to neurological damage from former acute polio.

初期の注意事項
As the First Step in Management

 ポリオ体験者は、神経の評価の専門で、ポリオについて詳しい医者による、十分な全身の健康診断を受けるべきです。
Polio survivors should undergo a complete, general medical evaluation by a primary care physician and a specialized neuromuscular evaluation by a knowledgeable and experienced polio specialist

 将来的な変化を予想し、適切な治療方針を立ててもらうためです。
to establish a baseline from which to judge future changes and to develop an appropriate treatment plan.

 ポリオ体験者は、第一に、自分の健康を定期的に、基本的な医学的な関心を持って、見続けるべきです。
Polio survivors should first and foremost take care of their health by seeking periodic, basic medical attention.

 栄養を十分に摂り、肥満を防ぎ、禁煙をし、アルコールに溺れないことです。
Be nutrition wise, avoid or reverse excessive weight gain, and stop smoking or over-indulging in alcohol.

 ポリオ体験者は、自分の体の言うことに耳を傾けるべきです。
Polio survivors should listen to their bodies.

 痛みを伴う運動は避けましょう。痛みは警告のシグナルです。
Avoid activities that cause pain - pain is a warning signal.

 痛み止めに頼りすぎるのは止めましょう。特に麻薬には。
Avoid unrestrained use of pain killers, especially narcotics.

 筋肉を使いすぎないようにしましょう。ただし、症状を悪化させない程度の運動は続けましょう。
Do not overuse muscles but do continue activity that does not worsen the symptoms.

 特に、痛みを感じる状態では、運動をしないようにしましょう。
In particular, do not over exercise or continue to exercise through pain.

 疲労感が10分以上続くような活動は、止めましょう。
Avoid activity that causes fatigue lasting more than ten minutes.

--------------------

 何らかの症状を持つ場合は、日常生活のペースを考えましょう。疲れたら、休みましょう。
Polio survivors experiencing symptoms should pace themselves in their daily activities, resting when tired.

 必要に応じて、1日に数回、15分から30分間、休みましょう。
Stop for a 15 to 30 minute rest when needed, perhaps several times a day.

 自分の体を管理するためには、補装具の使用も増やしましょう。
Management may include the increased use of assistive devices.

 機器や補装具を変えてみることもお勧めします。
A change of equipment or new bracing may be recommended.

 ポリオ体験者は、 自ら勉強するだけではなく、家族や他の人たち、もし必要であれば医療関係者にも、自分が知っていることを教えましょう。
Polio survivors should educate themselves, their families, and if need be, their health professionals.
