The most important person on your team is your Primary Care Physician, who will care for all of your basic medical needs.

ポストポリオ診断チームは、本部長で整形外科医のバートン・Ｗ・マーシュ医師が統括して、評価してくれます。
Evaluations by the
Post-Polio Evaluation Team will be performed by Team Director
Burton W. Marsh, M.D., P.A. (Board Certified Orthopaedic
Surgeon),
理学療法士、作業療法士、呼吸療法士、栄養士、矯正士のような、健康管理の専門家も、協力します。
along with a team of
healthcare specialists including Physical Therapists,
Occupational Therapists, Respiratory Therapists, Nutrition
Therapists, and Orthotists.
補装具、補助器具、療法の作成、栄養設計などについての、助言が得られるでしょう。
Recommendations may be made
for bracing, adaptive aids, therapy programs and/or a
nutritional program.
泌尿器科学、心臓学、神経学のような、他の医学的な評価も大切です。
Other medical services
(referral) may also be necessary: Pulmonology, Urology,
Cardiology or Neurology.

We also recommend you
download our 10-page Post-Polio Personal Information form, fill
it out, and present it to the physician who will be performing
your evaluation.

conducting your evaluation by answering nearly all questions the
physician may ask you.

If you do not plan to have
an evaluation immediately, we still recommend you fill out the
form and either present it when you go for an evaluation, go
under the care of a new physician, or keep it in your own records
until needed.
